Effectively July 1st, 2009, Cathedral Foundation of Jacksonville, Inc.will become the new Fiscal Agent for the Florida Self-Directed Care Program.

The Cathedral Foundation understands that a smooth transition is extremely important to all stakeholders of the SDC program. Our goal will be to transition functions in a manner that minimizes disruption of services or displacement of valuable SDC participants and staff.
