In order to be eligible to participate in FloridaSDC, an individual must meet the following criteria: 

·         Has been diagnosed with a mental illness

·         Be 18 years or older, and

·         Live in Clay, Nassau, Duval, and

·         Receive supplemental security income (SSI), Social Security disability income (SSDI), disabled veterans income, or any other type of disability income due to psychiatric disability; or

·         Receive Social Security income (SSI) for reasons other than psychiatric disability; or

·         Does not receive disability income due to psychiatric disability, but has an application in process or has received such income within the last 5 years; and

·         Is legally competent to direct his or her own affairs; and

·         Is not enrolled in Florida Assertive Community Treatment (FACT) or other case management services or plans to disenroll from this service; and

·         Applies for Medicaid and other income support programs for which he or she may qualify.
