I am Mark Moore, a healthcare professional who has provided consultant services, care management and training services in Duval, Clay, Nassau, and St. Johns counties for the past 16 years. You are all about choices, the freedom to choose, and the path to follow in life. I am here to be “someone to stand by you”, providing a nudge, lending an ear, and/or help find services or agencies allowing you to continue your journey with minimum interference.

While I have had extensive experience working with various age groups, their caregivers, and families, my “expertise” includes coordinating the steps necessary for self independence while conserving physical and financial resources, care assessment, and plan development with a multi-disciplinary healthcare team.

I am currently working on a special certification program through UF, which will enable me assist setting up and guiding physically challenged individuals by assisting in life care planning while maximizing resources, minimizing hurdles, and fostering independence.
